Driver's Education Form - turn it into the Wabash City Schools Administration office (189 W. Market St.)

*YOU ONLY NEED TO COMPLETE ONE ATTACHED ENROLLMENT OPTION ABOVE*

If you wish to enroll your student in the 2024 Driver’s Education Program, complete the attached application and return it to the Administration Office (189 W. Market St.) with $200.00 to secure your spot. At least $200.00 of the $400.00 fee must accompany your completed application, with the balance due on or before Monday, May 20th. Payment can be made via credit/debit card through our RevTrak Payment Portal (https://wabashcityschools.revtrak.net/) which can be found on our website under “Parents > RevTrak Payment Portal”. Click on “Driver’s Education” to make payments. If you are paying by check, make checks payable to Wabash High School. Please write your student’s name and “Driver’s Education” in the memo line and turn it into the Wabash City Schools Administration office (189 W. Market St.). **PAYMENTS WILL NOT BE ACCEPTED AT WABASH HIGH SCHOOL**
